Whether you do your own noise monitoring or have someone do it for you, you want to be sure the noise survey is performed and documented properly.  If you are inspected by OSHA, the compliance officer will want to review your noise survey results to be sure they are accurate and up to date.  The following checklist is provided to help you document your noise monitoring procedures and results.  Items generating "No" answers should be recorded, along with planned actions and proposed completion dates.  If you are unsure of an item, check "No" until the correct answer can be verified.  Once you have the noise exposure data you need, you can include them in this section or complete the Noise Survey Results Summary Record.
