Introduction
The Employee Respirator Assignment and Training Record is used to document the assignment of respirators and the training of respirator users, as recommended in ANSI Z88.2 and required in OSHA regulations.
Use
Once an evaluation of the respiratory hazards, other workplace factors, and a medical evaluation have been completed, a respirator or respirators can be assigned to individual employees. The Respirator Assignment and Training Record form can be used to record the assignment of up to two respirators for each employee. Based on the employee's job or activity, the assigned respirator should coincide with the respirator selected for the job or activity in the Hazard Evaluation and Respirator Selection Record. A more protective respirator can always be assigned as long as there is not a medical limitation on wearing the more protective device. In some cases, a medical recommendation may direct you to assign a powered air-purifying respirator, even though the hazard evaluation determined that a regular air-purifying respirator provides adequate protection against air contaminants.
To meet the respirator training requirements in ANSI Z88.2
and OSHA regulations, initial and annual retraining is completed
each year. In certain cases, more frequent retraining may
be needed. The form can be used to document completed training
for up to five (5) training sessions.
